How to get an EPA kit

Enduring power of attorney kits and guides can be found in the download section of this page. The EPA form, found in the back of the pdf versions, can be printed by selecting Print forms:

Hard copies of the enduring power of attorney kit may also be purchased from the Citizen’s Advice Bureau for a small fee. Contact your local branch for details.

Telephone Advisory Service

The Public Advocate provides an information service that can answer a range of common questions about enduring powers of attorney.

The Telephone Advisory Service includes recorded information that can be accessed 24 hours a day, seven days a week, by calling 1300 858 455. Advisory officers are available to speak to between 9am and 4:30pm, Monday to Friday.
